NEW PATIENT REMINDER

When Making your first appointment, please be sure to mention your insurance carrier. If we are not participating with the carrier, we will inform you so that you can be prepared to make future payment at the time of service. At your first appointment, it is important to bring your insurance card with you. If the visit is for a Workers Compensation claim, we will need the name of your employer, a contact's name, address, telephone number, and the name of your immediate supervisor. Please also have arrangements made for the transfer of your records from your previous physician.

For a specialist visit or special testing, please call our office one day in advance for further information or any special instructions.

GENERAL APPOINTMENT POLICIES

Patients are seen Monday through Friday, with varying hours for each office. Our offices are closed on Memorial Day, July 4th, Thanksgiving, Christmas, and New Years Day. If you are unable to keep your appointment, it must be cancelled 24 hours in advance. If you do not show up for your appointment and did not call to cancel, it is office policy to add a no-show charge of $20 to your account.

REFERRAL POLICY

Medcom Health Services wants to work with you and your insurance company.

Please use the following guidelines to receive the best service:

- There will be no out-of-plan referrals with any insurance.

- There will be no past-dated referrals and no service will be authorized after it is performed. To prevent such a situation, please contact our office 5 business days before the scheduled visit.
