When making your first appointment, please be sure to mention your insurance carrier. If we are not participating with the carrier, we will inform you so that you can be prepared to make future payment at the time of service. At your first appointment, it is important to bring your insurance card with you. If the visit is for a Workers Compensation claim, we will need the name of your employer, a contact’s name, address, telephone number, and the name of your immediate supervisor. Please also have arrangements made for the transfer of your records from your previous physician.

Patients are seen Monday through Friday, with varying hours for each office. Our offices are closed on Memorial Day, July 4th, Thanksgiving, Christmas, and New Years Day. If you are unable to keep your appointment, it must be cancelled 24 hours in advance. No show and cancelled appointments will have $20 charged to their account.

– No out-of-plan referrals will be accepted with any insurance.
– We cannot accept post dated referrals nor provide service unless the referral is current. To prevent such a situation, please contact our office 3 business days before the scheduled visit.
